In September 2021 Evangelista explained why she had withdrawn from public life and not been working. Five years previously she had undergone the cosmetic fat removal procedure cryolipolysis, under the brand name CoolSculpting, but that this had done the opposite with the complication paradoxical adipose hyperplasia (PAH). In 2023, Evangelista revealed that she had been diagnosed with breast cancer in December 2018. She received chemotherapy and underwent a bilateral mastectomy; the cancer returned in 2022 in her pectoral muscle for which she also received chemotherapy.
