Weapon Systems Explosives Safety Review Board
The United States Navy formed the Weapon System Explosives Safety Review Board (WSESRB) in 1967 as a result of two deadly accidents involving explosive ordnance aboard US aircraft carriers: the 1966 USS Oriskany fire, and the 1967 USS Forrestal fire On 29 July 1967, a fire broke out on board the aircraft carrier USS ''Forrestal'' after an electrical anomaly caused a Zuni rocket on an F-4B Phantom to fire, striking an external fuel tank of an A-4 Skyhawk. USS Oriskany Fire
The 1966 USS ''Oriskany'' fire was a major fire that broke out aboard the ''Essex''-class aircraft carrier on the morning of 26 October 1966. The fire broke out after a lit flare was locked in a flare locker. The fire killed 44 people, mostly air crew, and injured 156 more. It was the first of three major fires to befall American carriers during the Vietnam War. Background ''Oriskany'' departed San Diego for her second Vietnam deployment, and third wartime deployment, on 26 May 1966. The carrier arrived on station off the coast of Vietnam a few weeks later, and commenced operations in support of operations in Vietnam. 1967 USS Forrestal Fire
On 29 July 1967, a fire broke out on board the aircraft carrier USS ''Forrestal'' after an electrical anomaly caused a Zuni rocket on an F-4B Phantom to fire, striking an external fuel tank of an A-4 Skyhawk. The flammable jet fuel spilled across the flight deck, ignited, and triggered a chain reaction of explosions that killed 134 sailors and injured 161. At the time, ''Forrestal'' was engaged in combat operations in the Gulf of Tonkin, during the Vietnam War. The ship survived, but with damage exceeding US$72 million, not including the damage to aircraft. Future United States Senator John McCain and future four-star admiral and U.S. Pacific Fleet Commander Ronald J. Zlatoper were among the survivors. Another on-board officer, Lieutenant Tom Treanore, later returned to the ship as her commander and retired an admiral. Weapon System Safety
The system safety concept calls for a risk management strategy based on identification, analysis of hazards and application of remedial controls using a systems-based approach. This is different from traditional safety strategies which rely on control of conditions and causes of an accident based either on the epidemiological analysis or as a result of investigation of individual past accidents. The concept of system safety is useful in demonstrating adequacy of technologies when difficulties are faced with probabilistic risk analysis. The underlying principle is one of synergy: a whole is more than sum of its parts. Systems-based approach to safety requires the application of scientific, technical and managerial skills to hazard identification, hazard analysis, and elimination, control, or management of hazards throughout the life-cycle of a system, program, project or an activity or a product.
