Shoulder Presentation
In obstetrics, a shoulder presentation is a malpresentation at childbirth where the baby is in a transverse lie (its vertebral column is perpendicular to that of the mother), thus the leading part (the part that first enters the birth canal) is an arm, a shoulder, or the trunk. While a baby can be delivered vaginally when either the head or the feet/buttocks are the leading part, it usually cannot be expected to be delivered successfully with a shoulder presentation unless a cesarean section (C/S) is performed. Frequency and causes Shoulder presentations are uncommon (about 0.5% of births) since, usually, toward the end of gestation, either the head or the buttocks start to enter the upper part of the pelvis, anchoring the fetus in a longitudinal lie. William Smellie (obstetrician)
William Smellie (5 February 1697 – 5 March 1763) was a Scottish people, Scottish Obstetrics, obstetrician and medical instructor who practiced and taught primarily in London. One of the first prominent Midwife#Men in midwifery, male midwives in Britain, he designed an improved version of the obstetrical forceps, established safer delivery practices, and through his teaching and writing helped make obstetrics more scientifically based. He is often called the "father of British midwifery". Early life and education Smellie was born on 5 February 1697 in the town of Lesmahagow, Scotland. He was the only child of Sara Kennedy (1657–1727) and Archibald Smellie (1663/4–1735), a merchant and burgess of the town. Smellie practiced medicine before getting a license, opening an apothecary in 1720 in Lanark. Multiple Gestation
A multiple birth is the culmination of a multiple pregnancy, wherein the mother gives birth to two or more babies. A term most applicable to vertebrate species, multiple births occur in most kinds of mammals, with varying frequencies. Such births are often named according to the number of offspring, as in ''twins'' and ''triplets''. In non-humans, the whole group may also be referred to as a ''Litter (animal), litter'', and multiple births may be more common than single births. Multiple births in humans are the exception and can be exceptionally rare in the largest mammals. A multiple pregnancy may be the result of the fertilization of a single egg that then splits to create identical fetuses, or it may be the result of the fertilization of multiple eggs that create fraternal ("non-identical") fetuses, or it may be a combination of these factors.
