Pericardial Window Techniques
A pericardial window is a cardiac surgical procedure to create a fistula – or "window" – from the pericardial space to the pleural cavity. The purpose of the window is to allow a pericardial effusion or cardiac tamponade to drain from the space surrounding the heart into the chest cavity. Uses Pericardial window may be used to treat pericardial effusion and cardiac tamponade. It is the most common procedure to treat pericardial effusion, particularly if caused by cancer. Untreated, these can lead to death. The pericardial window decreases the incidence of postoperative pericardial tamponade and new-onset atrial fibrillation after open-heart surgery. Risks Creation of a pericardial window is a major surgical procedure. To remove pericardial fluid, other more minor techniques should be considered first, such as pericardiocentesis. Pericardiocentesis
Pericardiocentesis (PCC), also called pericardial tap, is a medical procedure where fluid is aspirated from the pericardium (the sac enveloping the heart). Anatomy and physiology The pericardium is a fibrous sac surrounding the heart composed of two layers: an inner visceral pericardium and an outer parietal pericardium. The area between these two layers is known as the pericardial space and normally contains 15 to 50 mL of serous fluid. This fluid protects the heart by serving as a shock absorber and provides lubrication to the heart during contraction. The elastic nature of the pericardium allows it to accommodate a small amount of extra fluid, roughly 80 to 120 mL, in the acute setting. However, once a critical volume is reached, even small amounts of extra fluid can rapidly increase pressure within the pericardium. Dominique Jean Larrey
Dominique Jean, Baron Larrey (8 July 1766 – 25 July 1842) was a French surgeon and soldier best known for his service in the French Revolutionary and Napoleonic Wars. An important innovator in battlefield medicine and triage, Larrey invented the flying ambulance and is sometimes considered the first modern military surgeon. Early life and career Larrey was Born in Beaudéan, and the second of three children to Jean Larrey, a shoemaker, and Philippine Perès. His father died in 1780, When Larrey was only 13 years old. He was then sent to live with his uncle Alexis, a surgeon in Toulouse where he learned his first medical skills. After an 8-year apprenticeship, he went to Paris to study under Pierre-Joseph Desault, who was chief surgeon at the Hôtel-Dieu de Paris. His uncle gave him a letter of introduction but money was scarce and Larrey walked all the way from Toulouse to Paris. Pericardium
The pericardium (: pericardia), also called pericardial sac, is a double-walled sac containing the heart and the roots of the great vessels. It has two layers, an outer layer made of strong inelastic connective tissue (fibrous pericardium), and an inner layer made of serous membrane (serous pericardium). It encloses the pericardial cavity, which contains pericardial fluid, and defines the middle mediastinum. It separates the heart from interference of other structures, protects it against infection and blunt trauma, and lubricates the heart's movements. Thoracoscopy is a medical procedure involving internal examination, biopsy and/or resection/drainage of disease or masses within the pleural cavity, usually with video assistance. Thoracoscopy may be performed either under general anaesthesia or under sedation with local anaesthetic. History Thoracoscopy was first performed by Sir Francis Cruise of the Mater Misericordiae Hospital in Dublin in conjunction with Dr Samuel Gordon in 1865. It was further developed by Hans Christian Jacobaeus, a Swedish internist in 1910 for the treatment of tuberculous intra-thoracic adhesions.
