Ockenden Review
In 2017 Rhiannon Davies, her husband Richard and two other bereaved parents, Kayleigh and Colin Griffiths asked the UK health secretary, Jeremy Hunt, to set up a public inquiry into maternity services at Shrewsbury and Telford Hospital NHS Trust. Though Hunt did not establish a public inquiry, he ordered a review in April 2017. In May 2017 Donna Ockenden was appointed chair of the review, and it initially investigated 23 cases of potentially significant concern . As the review uncovered wider evidence of maternity services failings at the trust, with over 60 cases needing investigation, Davies called on Hunt's successor, Matt Hancock, to widen the review's remit. Rhiannon Davies
Rhiannon Louise Davies and Richard Anthony Stanton are married British activists who worked to establish the truth about the death of their daughter, Kate Stanton-Davies, at Shrewsbury and Telford Hospital NHS Trust in 2009. Their efforts led to the establishment of the Ockenden Review of maternity services, led by Donna Ockenden. Further, their campaigning led to West Mercia Police instigating Operation Lincoln into both individual and corporate gross negligence manslaughter at Shrewsbury and Telford Hospital NHS Trust. Death of Kate Stanton-Davies In March 2009, Davies gave birth to a daughter, Kate Stanton-Davies, at a midwife-led maternity unit in Ludlow. Maternity Services
A maternity hospital specializes in caring for women during pregnancy and childbirth. It also provides care for newborn infants, and may act as a centre for clinical training in midwifery and obstetrics. Formerly known as lying-in hospitals, most of them, like cottage hospitals, have been absorbed into larger general hospitals, where they operate as the maternity department. History Maternity hospitals in the United Kingdom can be traced back to a number of 18th century establishments in London and Dublin. Prior to these foundations, childbirth was a domestic occasion. The term coined for these establishments, but now archaic, is "a lying-in hospital", referring to the custom of lying-in, prolonged bedrest after childbirth, better known now as postpartum confinement. Shrewsbury And Telford Hospital NHS Trust
Shrewsbury and Telford Hospital NHS Trust is the main provider of hospital services for Shropshire, Telford and Wrekin and North Powys. It runs the Royal Shrewsbury Hospital, the Princess Royal Hospital, Telford, Princess Royal Hospital in Telford, Oswestry Maternity Unit, and Wrekin Community Clinic, Euston House, Telford, in Shropshire, England. It is one of a small number of English NHS Trusts which takes patients from over the border in Wales. Donna Ockenden
Donna Ockenden , is a British midwife and community activist. She was commissioned in 2016 by the then UK Secretary for Health and Social Care, Jeremy Hunt, to chair an independent review into maternity services at Shrewsbury and Telford Hospital NHS Trust. Initial findings of the Ockenden Review were reported in December 2020, with a final report published on 17 March 2022. In May 2022, it was announced by NHS England that Ockenden would chair an independent review of maternity services at Nottingham University Hospitals NHS Trust (NUH). This review commenced at the beginning of September 2022 and is expected to continue until late 2025. In February 2021, Ockenden was named as a Fellow of the Royal Society for the Encouragement of Arts, Manufacture and Commerce. James Titcombe
James Roger Titcombe OBE is a patient safety specialist. He was previously, from October 2013 to March 2016, the National Advisor on Patient Safety, Culture & Quality for the Care Quality Commission. Earlier in his career he was project manager in the nuclear industry. His baby son Joshua died of sepsis at Freeman Hospital nine days after his birth at Furness General Hospital in November 2008. James refused to accept the initial explanations he and his wife were given by University Hospitals of Morecambe Bay NHS Trust for Joshua's death and made an official complaint about his treatment. Lady Hale
Brenda Marjorie Hale, Baroness Hale of Richmond, (born 31 January 1945), is a British judge who served as President of the Supreme Court of the United Kingdom from 2017 until her retirement in 2020.
