Middle Cerebellar Peduncle
The middle cerebellar peduncle (or brachium pontis) is one of three paired cerebellar peduncles connecting the brainstem to the cerebellum. The connection is from the pons. It connects the pons to the cerebellum, with fibres originating from the pontine nuclei, and travelling to the opposite cerebellar hemisphere. It is supplied by the anterior inferior cerebellar artery (AICA) and branches from the basilar artery. It conveys information from the cerebrum and the pons to the cerebellum. Structure The middle cerebellar peduncle is the largest of the three cerebellar peduncles. It connects the pons and cerebellum. It consists almost entirely of fibers passing from the pons to the cerebellum (fibrocerebellar fibers); the fibers arise from the pontine nuclei and decussate within the pons before entering the peduncle to end in the contralateral cerebellar hemisphere. The inferior cerebellar peduncle is formed by fibers of the restiform body that join with fibers from the much smaller juxtarestiform body. The inferior cerebellar peduncle is the smallest of the three cerebellar peduncles. In the human brain, the superior cerebellar peduncle (brachium conjunctivum) is one of the three paired cerebellar peduncles of bundled fibers that connect the cerebellum to the brainstem. The superior cerebellar peduncle connects to the midbrain. It consists mainly of efferent fibers, the cerebellothalamic tract that runs from a cerebellar hemisphere to the contralateral thalamus, and the cerebellorubral tract that runs from a cerebellar hemisphere to the red nucleus. It also contains afferent tracts, most prominent of which is the ventral spinocerebellar tract. Diffuse Intrinsic Pontine Glioma
Diffuse midline glioma, H3 K27-altered (DMG) is a tumour that arises in midline structures of the brain, most commonly the brainstem, thalamus and spinal cord. When located in the pons it is also known as diffuse intrinsic pontine glioma (DIPG). DMG is believed to be caused by Carcinogenesis#Multiple_mutations, genetic mutations that cause epigenetic changes in cells of the developing nervous system, resulting in a failure of the cells to properly differentiate. Currently, the standard of care is fractionated external beam radiotherapy, as the tumour location precludes surgery, and chemotherapy has shown to be ineffective.
