Gaspar Roque Francisco Narciso Casal Julian (31 December 1681 – 10 August 1759) was a Spanish physician remembered for describing the Casal collar in pellagra. Casal was born in Girona Catalonia, Spain to Federico Casal y Dajón and Magdalena Julian. He grew up in Utrillas and is thought to have apprenticed at Atienza with Juan Manuel Rodriguez de Lima, an apothecary to Pope Innocent XI. Casal Collar
The Casal collar or Casal necklace is a medical sign in which there is a redness and darkening of the skin around the neck, seen in people with pellagra. It becomes prominent following exposure to sun, and can be itchy with a burning feeling and pain. It can blister and weep, and turn into thick dark skin, with a coppery hue. If prolonged, the skin becomes thin, dry and smooth. The sign is caused by a lack of the niacin (vitamin B3). Seborrheic dermatitis may appear similar. Pellagra
Pellagra is a disease caused by a lack of the vitamin niacin (vitamin B3). Symptoms include inflamed skin, diarrhea, dementia, and sores in the mouth. Areas of the skin exposed to friction and radiation are typically affected first. Over time affected skin may become darker, stiffen, peel, or bleed. There are two main types of pellagra, primary and secondary. Primary pellagra is due to a diet that does not contain enough niacin and tryptophan. Secondary pellagra is due to a poor ability to use the niacin within the diet. This can occur as a result of alcoholism, long-term diarrhea, carcinoid syndrome, Hartnup disease, and a number of medications such as isoniazid. Diagnosis is typically based on symptoms and may be assisted by urine testing. Treatment is with either nicotinic acid or nicotinamide supplementation. Pope Innocent XI (; ; 16 May 1611 – 12 August 1689), born Benedetto Odescalchi, was head of the Catholic Church and ruler of the Papal States from 21 September 1676 until his death on 12 August 1689. Political and religious tensions with Louis XIV of France were a constant preoccupation for Innocent XI. Ferdinand VI (; 23 September 1713 – 10 August 1759), called the Learned (''el Prudente'') and the Just (''el Justo''), was King of Spain from 9 July 1746 until his death in 1759.
