In the United Kingdom, Ireland, and parts of the Commonwealth, consultant is the title of a senior hospital-based physician or surgeon who has completed all of their specialist training and been placed on the specialist register in their chosen speciality. The primary objective of a consultant is to use expert knowledge and skill to diagnose and treat patients and ultimate clinical responsibility for their care. A physician must be on the Specialist Register before they may be employed as a substantive consultant in the National Health Service (NHS). Foundation Doctor
A foundation doctor is a grade of medical practitioner in the United Kingdom undertaking the Foundation Programme, a two-year, general postgraduate medical training programme which forms the bridge between medical school and specialist/general practice training. Doctors in the first year of the programme are known as Foundation Year 1 (FY1) doctors, and those in the second year are known as Foundation Year 2 (FY2) doctors. Being a foundation doctor is compulsory for all newly qualified medical practitioners in the UK starting from 2005 onwards. Clinical Excellence Awards
National Clinical Impact Awards are awarded within the English National Health Service to consultants and academic GPs who perform 'over and above' the standard expected of their role. In January 2022 reforms were announced. There will be more awards but at lower levels. There are 3 levels rather than 4. They will still run for 5 years. There will be up to 600 awards granted each year with 70 national 3 awards worth £40,000 per year, 200 national 2 at £30,000 per year and 330 national 1 awards at £20,000 per year. For the first time those who work less than full time, mostly women, will get the full value instead of pro-rata. Local award schemes, managed by individual employers, are not affected. Modernising Medical Careers
Modernising Medical Careers (MMC) is a programme for postgraduate medical training introduced in the United Kingdom in 2005. The programme replaced the traditional grades of medical career before the level of Consultant. The different stages of the programme contribute towards a " Certificate of Completion of Training" (CCT). A specialty registrar (StR), previously known as and still commonly referred to as a specialist registrar (SpR), is a doctor, public health practitioner or dentist who is working as part of a specialty training programme in the UK. This is known as a training grade as these doctors are supervised to an extent, as part of a structured training experience that leads to being able to undertake independent practice in a hospital specialty or working as a general practitioner.
