Urogenital Hiatus
The urogenital hiatus is a large midline opening in the anteromedial part of the pelvic floor (more precisely, the Pubococcygeal (PC) muscles, pubococcygeal muscle), extending between the pubis (anteriorly), and rectum (posteriorly). Each Levator ani, levator ani muscle forms either lateral border of the hiatus. The hiatus acccomodates the apex of the prostate in males, and gives passage to the urethra in both sexes, the vagina in females, the deep dorsal vein of clitoris (females) or Deep dorsal vein of penis, penis (males), and nerves of the penis in males. __TOC__ Clinical significance The urogenital hiatus has been linked to urinary stress incontinence. Pelvic Floor
The pelvic floor or pelvic diaphragm is an anatomical location in the human body which has an important role in urinary and anal continence, sexual function, and support of the pelvic organs. The pelvic floor includes muscles, both skeletal and smooth, ligaments, and fascia and separates between the pelvic cavity from above, and the perineum from below. It is formed by the levator ani, levator ani muscle and coccygeus muscle, and associated connective tissue. The pelvic floor has two hiatus (anatomy), hiatuses (gaps): (anteriorly) the urogenital hiatus through which urethra and vagina pass, and (posteriorly) the rectal hiatus through which the anal canal passes. The levator ani is a broad, thin muscle group, situated on either side of the pelvis. It is formed from three muscle components: the pubococcygeus, the iliococcygeus, and the puborectalis. It is attached to the inner surface of each side of the lesser pelvis, and these unite to form the greater part of the pelvic floor. The coccygeus muscle completes the pelvic floor, which is also called the ''pelvic diaphragm''. It supports the viscera in the pelvic cavity, and surrounds the various structures that pass through it. Pelvic floor dysfunction is a term used for a variety of disorders that occur when pelvic floor muscles and ligaments are impaired. The condition affects up to 50 percent of women who have given birth. Although this condition predominantly affects women, up to 16 percent of men are affected as well. The perineologist takes a holistic approach, using defect-specific and mini-invasive treatments. History of the term The word perineology represents a neologism. It was invented for the occasion of the 1st International Symposium on Perineology, held in Venice in 1990 to reflect an integrated view of the anatomy, physiology and pathology of the pelvic floor. It was popularized by the Italian journal Rivista Italiana di Colon-Proctologia. Perineal hernia is a hernia involving the perineum (pelvic floor). The hernia may contain fluid, fat, any part of the intestine, the rectum, or the urinary bladder, bladder.
