Vitrectomy is a surgery to remove some or all of the vitreous humor from the Human eye, eye. Anterior vitrectomy entails removing small portions of the vitreous humor from the front structures of the eye—often because these are tangled in an intraocular lens or other structures. ''Pars plana'' vitrectomy is a general term for a group of operations accomplished in the deeper part of the eye, all of which involve removing some or all of the vitreous humor—the eye's clear internal jelly. Even before the modern era, some surgeons performed crude vitrectomies. Robert Machemer (16 March 1933 in Münster – 23 December 2009 in Durham, North Carolina) was a German-American ophthalmologist, ophthalmic surgeon, and inventor. He is sometimes called the "father of modern retinal surgery." Helmut Machemer, Robert's father, was an ophthalmologist who died in Ukraine on 18 May 1942, leaving a widow and three small sons. Retinal detachment is a condition where the retina pulls away from the tissue underneath it. It may start in a small area, but without quick treatment, it can spread across the entire retina, leading to serious vision loss and possibly blindness. Retinal detachment is a medical emergency that requires surgery. The retina is a thin layer at the back of the eye that processes visual information and sends it to the brain. When the retina detaches, common symptoms include seeing floaters, flashing lights, a dark shadow in vision, and sudden blurry vision. A scleral buckle is one of several ophthalmologic procedures that can be used to repair a retinal detachment. Retinal detachments are usually caused by retinal tears, and a scleral buckle can be used to close the retinal break, both for acute and chronic retinal detachments. Scleral buckles come in many shapes and sizes. A silicone sponge (with air filled cells) is a cylindrical element that comes in various sizes. An encircling band is a thin silicone band sewn around the circumference of the sclera of the eye. A solid silicone grooved tyre element is also used. Buckles are often placed under a band to create a dimple on the eye wall. The scleral buckle is secured around the eyeball under the conjunctiva. The macula in humans has a diameter of around and is subdivided into the umbo, foveola, foveal avascular zone, fovea, parafovea, and perifovea areas. Epiretinal membrane or macular pucker is a disease of the eye in response to changes in the vitreous humor or more rarely, diabetes. Sometimes, as a result of immune system response to protect the retina, cells converge in the macular area as the vitreous ages and pulls away in posterior vitreous detachment (PVD). PVD can create minor damage to the retina, stimulating exudate, inflammation, and White blood cell, leucocyte response. These cells can form a transparent layer gradually and, like all scar, scar tissue, tighten to create tension on the retina which may bulge and pucker, or even cause swelling or macular edema. Cataract surgery, also called lens replacement surgery, is the removal of the natural lens (anatomy), lens of the human eye, eye that has developed a cataract, an opaque or cloudy area.
