Sutton Homes Of Care
Sutton Homes of Care is one of six vanguard projects focussed on ‘Enhanced health in care homes’ established under the Five Year Forward View in 2015. It is a partnership involving Sutton Clinical Commissioning Group, Epsom and St Helier University Hospitals NHS Trust, Sutton Health Community Services, London Ambulance Service, South West London and St George's Mental Health NHS Trust, the London Borough of Sutton, Age UK Sutton, St Raphael's Hospice, Alzheimer's Society and Sutton Care for the Voluntary Sector. It has established a number of initiatives intended to keep residents of the 70 care homes in the borough out of hospital, or to get out of hospital more easily. These include regular named-GP visits to local care homes to provide healthcare, health and wellbeing reviews and end-of-life planning. It claims credit for a 10% drop in visits to A&E. Five Year Forward View
The Five Year Forward View was produced by NHS England in October 2014 under the leadership of Simon Stevens as a planning document. Publication and reception It received praise for brevity, being only 39 pages, and lacking the illustrations which had graced its predecessors. Like the NHS Plan 2000 with which Stevens was also associated it was supported by the great and good of the NHS, but in this case it was regulators - Monitor, the Care Quality Commission and the like, rather than the Royal Colleges and Trades Unions of the earlier plan. Clinical Commissioning Group
Clinical commissioning groups (CCGs) were NHS organisations set up by the Health and Social Care Act 2012 to organise the delivery of NHS services in each of their local areas in England. On 1 July 2022 they were abolished and replaced by Integrated care systems as a result of the Health and Care Act 2022. Establishment The announcement that GPs would take over this commissioning role was made in the 2010 white paper "Equity and Excellence: Liberating the NHS". This was part of the government's stated desire to create a clinically-driven commissioning system that was more sensitive to the needs of patients. Epsom and St Helier University Hospitals NHS Trust is an NHS trust covering the London Borough of Sutton and north Surrey. It runs two main hospitals: Epsom Hospital and St Helier Hospital. In England, a sustainability and transformation plan (STP) is a non-statutory requirement which promotes integrated provision of healthcare, including purchasing and commissioning, within each geographical area of the National Health Service.
