Mixed Transcortical Aphasia
Mixed transcortical aphasia is the least common of the three transcortical aphasias (behind transcortical motor aphasia and transcortical sensory aphasia, respectively). This type of aphasia can also be referred to as "Isolation Aphasia". This type of aphasia is a result of damage that isolates the language areas ( Broca's, Wernicke’s, and the arcuate fasciculus) from other brain regions. Broca's, Wernicke's, and the arcuate fasiculus are left intact; however, they are isolated from other brain regions. A stroke is one of the leading causes of disability in the United States. Following a stroke, 40% of stroke patients are left with moderate functional impairment and 15% to 30% have a severe disability as a result of a stroke. Watershed Stroke
A watershed stroke is defined as a brain ischemia that is localized to the vulnerable border zones between the tissues supplied by the anterior, posterior and middle cerebral arteries. The actual blood stream blockage/restriction site can be located far away from the infarcts. Watershed locations are those border-zone regions in the brain supplied by the major cerebral arteries where blood supply is decreased. Watershed strokes are a concern because they comprise approximately 10% of all ischemic stroke cases. Boston Diagnostic Aphasia Examination
The Boston Diagnostic Aphasia Examination is a neuropsychological battery used to evaluate adults suspected of having aphasia, and is currently in its third edition. It was created by Harold Goodglass and Edith Kaplan. The exam evaluates language skills based on perceptual modalities (auditory, visual, and gestural), processing functions (comprehension, analysis, problem-solving), and response modalities (writing, articulation, and manipulation). Administration time ranges from 20 to 45 minutes for the shortened version but it can last up to 120 minutes for the extended version of the assessment. There are five subtests which include: conversational & expository speech, auditory comprehension, oral expression, reading, and writing. Western Aphasia Battery
Western Aphasia Battery (WAB) is an instrument for assessing the language function of adults with suspected neurological disorders as a result of a stroke, head injury, or dementia. There is an updated version, the Western Aphasia Battery-Revised (WAB-R). It helps discern the presence, degree, and type of aphasia. It also measures how the patient performed on the test to provide a baseline so they can detect changes throughout their time in therapy. This also allows to see the patient's language strengths and weaknesses so that they can figure out what to treat, and lastly, it can infer the location of the lesion that caused aphasia. Wernicke's area (; ), also called Wernicke's speech area, is one of the two parts of the cerebral cortex that are linked to speech, the other being Broca's area. It is involved in the comprehension of written and spoken language, in contrast to Broca's area, which is primarily involved in the production of language. Aphasia is an inability to comprehend or formulate language because of damage to specific brain regions. The major causes are stroke and head trauma; prevalence is hard to determine but aphasia due to stroke is estimated to be 0.1–0.4% in the Global North. Aphasia can also be the result of brain tumors, brain infections, or neurodegenerative diseases (such as dementias). To be diagnosed with aphasia, a person's speech or language must be significantly impaired in one (or more) of the four aspects of communication following acquired brain injury.
