A health risk assessment (also referred to as a health risk appraisal and health & well-being assessment) is a questionnaire about a person's medical history, demographic characteristics and lifestyle. It is one of the most widely used screening tools in the field of health promotion and is often the first step in multi-component health promotion programs. Screening (medicine)
In medicine, screening is a strategy used to look for as-yet-unrecognised conditions or risk markers. This testing can be applied to individuals or to a whole population without symptoms or signs of the disease being screened. The Alameda County Study is a longitudinal study of residents from Alameda County, California, which examines the relationship between lifestyle and health. The "1965 cohort" were given health questionnaires in 1965, 1973, 1985, 1988, 1994, and 1999.
