Phaeohyphomycosis
Phaeohyphomycosis is a diverse group of fungal infections, caused by dematiaceous fungi whose morphologic characteristics in tissue include hyphae, yeast-like cells, or a combination of these. It can be associated with an array of melanistic filamentous fungi including '' Alternaria'' species, '' Exophiala jeanselmei'', and '' Rhinocladiella mackenziei''. The term "phaeohyphomycosis" was introduced to determine infections caused by dematiaceous (pigmented) filamentous fungi which contain melanin in their cell walls. Phaeohyphomycosis is an uncommon infection, but the number of cases reported has been increasing in recent years. Fungal melanin is thought to be a virulence factor. The outcome of antifungal treatment is poor, and mortality is almost 80%. Phaeohyphomycosis has been attributed to more than 100 species and 60 genera of fungi over the past several decades. Rhinocladiella Mackenziei
''Rhinocladiella mackenziei'' is a deeply pigmented mold that is a common cause of human cerebral phaeohyphomycosis. ''Rhinocladiella mackenziei'' was believed to be endemic solely to the Middle East, due to the first cases of infection being limited to the region. However, cases of ''R. mackenziei'' infection are increasingly reported from regions outside the Middle East. This pathogen is unique in that the majority of cases have been reported from immunologically normal people. History ''Rhinocladiella mackenziei'' was first identified in 1993 as ''Ramichloridium mackenziei'' by C.K. Campbell & Al-Hedaithy when it was identified as the cause of eight cases of human cerebral phaeohyphomycosis. All eight patients had abscess formation and six of the patients had aspirated pus with branching hyphae. Exophiala Jeanselmei
''Exophiala jeanselmei'' is a saprotrophic fungus in the family Herpotrichiellaceae. Four varieties have been discovered: ''Exophiala jeanselmei'' var. ''heteromorpha'', ''E. jeanselmei'' var. ''lecanii-corni'', ''E. jeanselmei'' var. ''jeanselmei'', and ''E. jeanselmei var. castellanii''. Other species in the genus ''Exophiala'' such as '' E. dermatitidis'' and '' E. spinifera'' have been reported to have similar annellidic conidiogenesis and may therefore be difficult to differentiate. History ''Exophiala jeanselmei'' was first isolated in 1928 by Jeanselme from a case of black mycetoma on the foot. The nomenclature was based on the fungus' morphological characteristics, hence, it was originally classified as ''Torula jeanselmei'' because of its yeast like shape when grown in culture. An opportunistic infection is an infection that occurs most commonly in individuals with an immunodeficiency disorder and acts more severe on those with a weakened immune system. Chromoblastomycosis is a long-term mycosis, fungal infection of the skin and subcutaneous tissue (a chronic (medicine), chronic subcutaneous mycosis). It can be caused by many different types of fungi which become implanted under the skin, often by thorns or splinters. Chromoblastomycosis spreads very slowly. Amphotericin B
Amphotericin B is an antifungal medication used for serious fungal infections and leishmaniasis. The fungal infections it is used to treat include mucormycosis, aspergillosis, blastomycosis, candidiasis, coccidioidomycosis, and cryptococcosis. For certain infections it is given with flucytosine. It is typically given intravenously. Common side effects include a reaction with fever, chills, and headaches soon after the medication is given, as well as kidney problems. Allergic symptoms including anaphylaxis may occur. Other serious side effects include low blood potassium and myocarditis (inflammation of the heart). It appears to be relatively safe in pregnancy.
