Engineering disasters often arise from shortcuts in the design process. Engineering is the science and technology used to meet the needs and demands of society. These demands include buildings, aircraft, watercraft, vessels, and computer software. In order to meet society’s demands, the creation of newer technology and infrastructure must be met efficiently and cost-effectively. To accomplish this, managers and engineers need a mutual approach to the specified demand at hand. This can lead to shortcuts in engineering design to reduce costs of construction and fabrication. Hurricane Katrina was a powerful, devastating and historic tropical cyclone that caused 1,392 fatalities and damages estimated at $125 billion in late August 2005, particularly in the city of New Orleans and its surrounding area. It is tied with Hurricane Harvey as being the List of the costliest tropical cyclones, costliest tropical cyclone in the Atlantic basin. Katrina was the twelfth tropical cyclone, the fifth hurricane, and the third major hurricane of the 2005 Atlantic hurricane season. Ethiopian Airlines Flight 302 was a scheduled international passenger flight from Bole International Airport in Addis Ababa, Ethiopia, to Jomo Kenyatta International Airport in Nairobi, Kenya. On 10 March 2019, the Boeing 737 MAX 8 aircraft which operated the flight crashed near the town of Bishoftu six minutes after takeoff. All 149 passengers and 8 crew members on board died. ET 302 is Ethiopian Airlines' deadliest accident to date, surpassing the fatal hijacking of Flight 961 resulting in a crash near the Comoros in 1996. Lion Air Flight 610 was a scheduled domestic passenger flight from Soekarno–Hatta International Airport, Tangerang, to Depati Amir Airport, Pangkal Pinang, in Indonesia. On 29 October 2018, the Boeing 737 MAX 8 operating the route, carrying 181 passengers and 8 crew members, crashed into the Java Sea 13 minutes after takeoff, killing all 189 occupants on board. The MIM-104 Patriot is a mobile interceptor missile surface-to-air missile (SAM) system, the primary such system used by the United States Army and several allied states. The Therac-25 is a computer-controlled radiation therapy machine produced by Atomic Energy of Canada Limited (AECL) in 1982 after the Therac-6 and Therac-20 units (the earlier units had been produced in partnership with of France). The Therac-25 was involved in at least six accidents between 1985 and 1987, in which some patients were given massive overdoses of radiation. Ariane Flight V88
Ariane flight V88 was the failed maiden flight of the Arianespace Ariane 5 rocket, vehicle no. 501, on 4 June 1996. It carried the Cluster spacecraft, a constellation of four European Space Agency research satellites. The launch ended in failure due to multiple errors in the software design: dead code, intended only for Ariane 4, with inadequate protection against integer overflow led to an exception handled inappropriately, halting the whole otherwise unaffected inertial navigation system. This caused the rocket to veer off its flight path 37 seconds after launch, beginning to disintegrate under high aerodynamic forces, and finally self-destructing via its automated flight termination system.
